Some of our physicians participate with the following insurances. Please call your insurance company to verify participation.
We provide insurance billing. Anything not covered by insurance will be your responsibility. We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan. If you have any questions regarding which insurance plans we accept or any patient billing concerns, please call us at the phone number below. Questions regarding your coverage and benefits should be directed to your employer or insurance company. For any business or billing questions or concerns, you can contact our business office directly at 719.389.1137. The business office hours are from 8:00 a.m. to 5:00 p.m. Monday through Friday. The business office phone hours are from 9:00 a.m. to 12:00 p.m. and from 1:00 p.m. to 4:00 p.m., Monday through Friday. The direct fax number to this department is 719.389.1197. Please request prescription refills during the following hours: Monday through Thursday from 9:00 a.m. to 4:00 p.m. and Friday from 9:00 a.m. to noon. If you request a prescription refill by telephone, we ask that you allow one to two days for the prescription to be filled. Please note that if your physician is out of the office due to vacation, etc., it may take longer than one to two days to refill your prescription. Our pharmacy line is 719.389.1189. A physician is available 24 hours a day for emergencies. Your call will be answered 24 hours a day, either by our office staff or the answering service.
